We’ve found that one of the easiest ways to have successful patient relationships is to explain everything in advance. No surprises!
Your First Appointment
Please bring your New Patient Paperwork (click to download, print and fill out at home or pick up the documents at our office) with you on your first appointment along with your insurance card and a picture ID. Your first visit will include a consultation with your doctor. The purpose of this consultation is to discuss the short an/or long term treatment plan options best for you. Consultations generally last about an hour. Once the consultation is complete, you will receive treatment during your first appointment.
Follow UP Visits
After the investment of time needed on the first visit, many patients become concerned that every visit will be of equal length. A typical follow up visit will last approximately 10-15 minutes. Due to our caregivers training and the medical information on-hand from previous visits, this is all of the time needed to provide you with world-class musculoskeletal care.
Case Chiropractic recognizes your time is valuable, therefore we do everything possible to stay on time and minimize delays.

Q: Where do I find the New Patient Paperwork?
A: You can download the New Patient Paperwork by clicking here or you pick up the documents at our office prior to your first visit.
Q: How will I be expected to pay for my appointments?
A: Depending on your eligibility from your insurance carrier. If your deductible is NOT met you will pay the allowable amount according to your insurance, copay, coinsurance also applies. There are some procedures that are not covered by your insurance that will be patient responsibility. An estimated "due from patient" will be expected upon appointment date.
Q: If I NO SHOW for an appointment will I be charged?
A: Yes we charge $50.00 for new patients and $25.00 for established patients, that do not show for their appointment.
Q: What is your cancellation policy?
A: We request 24 hours notice for a cancellation so that we may move patients up from our cancellation list.
Q: What forms of payment are accepted by Case Chiropractic?
A: We accept Credit Cards (Visa, MasterCard, American Express, Discover), Cash and Checks.
